Scars develop after skin has been traumatized from surgery, disease, an accident or a skin condition. Some are more conspicuous than others. They can be flat, raised, stretched and depressed. A scar’s prominence depends on its location, size and cause, as well as a person’s age, heredity and ethnicity. While a scar can diminish for up to two years, it can leave the one who bears it mentally, emotionally and physically uncomfortable.

Surgery can change a scar’s shape and position, as well as free a tight scar that’s near a joint whose movement is hindered by the scar’s location.
When a scar is deleted or revised, the scar tissue is excised and a nearby flap of healthy skin is used to reconstruct the area. If a nearby flap isn’t feasible and the scar is large, the surgeon can harvest a skin graft from another area of your body—frequently a buttock or inner thigh because the area is the most inconspicuous.
Another alternative is called Z-Plasty. It surgically reopens a linear scar and repositions the tissue into a more jagged appearance with more relaxed skin tension lines on the scar tissue so it’s less visually noticeable. In other words, Z-Plasty moves a scar into a natural fold or crease to minimize its visibility. It can also help relax or release tightness caused by scar tissue.
The use of tissue expanders is an additional approach. An expandable silicone implant is positioned under the skin. The healthy, stretched skin will eventually replace the excised scar tissue.
Whether you’ll need local or general anesthesia during scar-revision surgery depends on the scar’s location and size.
